Uninsured:

We do accept self-pay patients. For your visit we will take 20% off your office visit, if paid at the time of visit. State provided immunizations will be provided at no cost to you except for the administration fee.

Billing:

The following information is provided to answer frequently asked questions:

  • At the time of each visit please have your insurance card available. If we are unable to verify your child’s insurance you may be asked to pay for the visit.
  • We collect co pays at the time of visit and file a claim with the insurance company. We accept Visa, MasterCard, and Discovery.
  • Should there be a balance for a visit that is unpaid by the insurance company it is the responsibility of the parent/guardian to settle the balance.
  • If we don’t contract with your insurance plan we ask that you pay in full at the time of visit.
  • In cases of separation or divorce the parent bringing the child to the visit is responsible for payment at time of service.
  • Regardless of what a divorce decree states it is the responsibility of the parent bringing the child in to make payment. Payment must be resolved between the two parents.
